Supporting Documents for Certification

Upon submitting the online form, we ask that you start to gather supporting documents to accompany your certification application. The documents requested give the NGLCC certification committees and NGLCC staff a complete picture of your company. By submitting a complete application, you will ensure your application is processed as efficiently as possible.

All applicants must submit the following general business information in addition to documents specific to your business' legal structure. 

For items specific to the legal structure click your corresponding structure below:

 Sole Proprietors  Partnerships Corporations (S & C)    Limited Liability Company (LLC)

Items for ALL business types:

Administrative Items

- Printed Copy of the Online Application Form
- Signed and Properly Notarized Affidavit (Original)
- Non-refundable processing fee

Principals' Information

- Proof of U.S. Citizenship or Resident Alien Status

  • Passport (Valid) or
  • Birth Certificate or
  • Green Card

- Letter from two (2) personal references attesting to LGBT status of business owner(s)

- Applicant may submit a minimum of one (1) LGBT Status Qualifier from Group A to satisfy the requirement for proof of Sexual Orientation or Gender Identity of Owners, Partners or Shareholders. If applicant cannot prove status with qualifiers listed in Group A, applicant may submit a minimum of two (2) LGBT Status Qualifiers from Group B . If applicant cannot prove status with qualifiers listed in Group A, and possesses only one (1) qualifier from Group B, an additional qualifier from Group C may be submitted.

General Business Information
(All businesses submit unless start up. See section below if in business less than 1 year)

- Brief History of Business
- Applicable Operating Business License and/or Permits
- Previous year Financial Statement (Including, Profit & Loss Statement and Balance Sheet)
- SIGNED/COMPLETE Federal tax return forms from the past three (3) years where business income is reported
- List of Employees
- Including Name, Full/Part Time Status, and length of service
- Most recent Itemized Payroll (if applicable)
- Resume(s) of owner(s), partners, or shareholders
- W-2s or 1099 forms for every officer, director, or owner receiving compensation
- Service Agreements (if applicable)
- Affiliate/Subsidiary Agreements (if applicable)
- Equipment Rental and Purchase Agreements (if applicable)
- Contract or work history for the past three years - Include contact name, and type of work performed or type of contract received (if applicable)
- Management Agreements (if applicable)
- Other Third Party Agreements (if applicable)
- Real Estate Lease Agreements (if applicable)
- Proof of Bonding Capacity (if applicable)
- Current Union Agreements (if applicable)

Start-up Businesses Submit (if in business for less than 1 year):

- Brief business plan/overview (Detailing target audiences, income projections, service/commodity description, etc.)
- Opening balance sheet
- Three years' signed/complete tax returns where proprietors' income is reported

For documents that do not apply to your company, please provide a letter listing which documents do not apply and why.


The NGLCC understands that the confidentiality of the information you provide in connection with your application for certification is of the highest importance. The NGLCC, the NGLCC Certification Committee members and Site Visitors hold this information in the strictest confidence and each member with access to your information has pledged not to disclose or disseminate that information unless ordered by a court of competent jurisdiction or the government.
