Supporting Documents for Certification: LGBT Status Qualifiers - Group B

If applicant cannot prove status with qualifiers listed in Group A, applicant may submit a minimum of two (2) LGBT Status Qualifiers from Group B (below) to satisfy the requirement for proof of sexual orientation or gender identity of Owners, Partners, or Shareholders.

- Apartment lease with name of applicant and partner listed
- Automobile title, financing agreement, and/or insurance records with name of applicant and partner listed
- Cellular and/or home telephone service plans with name of applicant and partner listed
- Major loans and lines of credit with name of applicant and partner listed
- Records of philanthropic contributions by applicant and partner - Submit: tax donation form from receiving party with name of applicant and partner listed
- Renter's insurance, homeowner's insurance, life insurance, or other insurance plans
- Retirement plan documentation listing applicant's same-sex partner as a beneficiary
- Utility bills with name of applicant and partner listed
- Documentation showing investment holdings that are joint-owned by applicant and same-sex partner
- Proof of joint will for applicant and same-sex partner - Reference to partner must be made in the will.
- Clergy letter attesting to the LGBT status of the applicant

 

If applicant cannot prove status with qualifiers listed in Group A , and possesses only one (1) qualifier from Group B an additional qualifier from Group C.
