Supporting Documents for Certification: LGBT Status Qualifiers - Group C

If applicant cannot prove status with qualifiers listed in Group A , and possesses only one (1) qualifier from Group B an additional qualifier from Group C can be submitted to satisfy the requirement for proof of sexual orientation or gender identity of Owners, Partners, or Shareholders.

- Witten history of real or perceived discrimination experienced due to LGBT status of the applicant. (Applicant is encouraged to site business-related discrimination, although personal discrimination experienced may be included in written history.)
- Receipts from LGBT events. Examples: Examples include but are not limited to: Gay Day at Disney World, Orlando, FL, RSVP, Atlantis, Olivia or Sweet Vacations, Dinah Shore Weekend Event - Registration documents and receipts listing identification information of the applicant must be submitted to fulfill this qualifier. Information listed should at a minimum include name, address, and contact information. Additional information such as social security number, passport number, credit card and payment information or state identification numbers may also be cited on the documents as proof of participation and registration.
